مقاومت فنوتیپی به سیپروفلوکساسین و شناسایی ژن‌های gyrA و parC در Enterococcus faecalis جداشده از عفونت‌های دستگاه ادراری در بغداد، عراق

چکیده
هدف: یکی از علل مهم عفونت‌های دستگاه ادراری، باکتری Enterococcus faecalis است. این موضوع به‌ویژه در بیماران بستری در بیمارستان اهمیت دارد. نشان داده شده است که مقاومت E. faecalis به سیپروفلوکساسین می‌تواند در اثر تغییرات در ژن‌های کروموزومی کدکننده DNA ژیراز و توپویزومراز IV و همچنین سایر مکانیسم‌های مقاومت ایجاد شود. بنابراین، هدف این مطالعه تعیین الگوی حساسیت به سیپروفلوکساسین در جدایه‌های E. faecalis جداشده از نمونه‌های ادرار بود. همچنین، شناسایی ژن‌های gyrA و parC مرتبط با اهداف کینولون‌ها، به‌عنوان هدف دیگر این مطالعه در نظر گرفته شد.
مواد و روش‌ها: در ابتدا، 400 نمونه ادرار از سه بیمارستان در بغداد جمع‌آوری شد. از میان این 400 نمونه، 95 جدایه به‌عنوان Enterococcus faecalis شناسایی شدند. از میان این 95 جدایه، 20 جدایه (21.1%) به سیپروفلوکساسین مقاوم بودند. برای تأیید شناسایی، از سیستم VITEK 2 و روش PCR استفاده شد. حداقل غلظت مهاری (MIC) سیپروفلوکساسین بر اساس توصیه‌های CLSI ارزیابی شد. ژن‌های gyrA و parC تحت تکثیر قرار گرفتند و برای بررسی برهم‌کنش سیپروفلوکساسین با DNA ژیراز و توپویزومراز IV، داکینگ in silico انجام شد.
نتایج: از میان 400 نمونه ادرار، 95 جدایه Enterococcus faecalis شناسایی شدند. بیست جدایه (21.1%) دارای مقادیر MIC سیپروفلوکساسین در محدوده 64 تا 512 میکروگرم بر میلی‌لیتر بودند، درحالی‌که 75 جدایه باقی‌مانده (78.9%) مقادیر MIC بین 2 تا 4 میکروگرم بر میلی‌لیتر داشتند. وجود ژن‌های gyrA و parC در جدایه‌های مقاوم به سیپروفلوکساسین با استفاده از PCR تأیید شد. نتایج نشان داد که امتیاز پیش‌بینی‌شده داکینگ از 8/7- کیلوکالری بر مول در مدل نوع وحشی به 89/3- کیلوکالری بر مول در مدل جهش‌یافته تغییر کرد. این تغییرات نشان‌دهنده ایجاد یک برهم‌کنش پیش‌بینی‌شده نامطلوب‌تر بین سیپروفلوکساسین و گیرنده جهش‌یافته است. نتایج تحلیل داکینگ نشان داد که برهم‌کنش‌های پیش‌بینی‌شده سیپروفلوکساسین با پروتئین‌های مدل‌شده جهش‌یافته، در مقایسه با مدل‌های نوع وحشی، مطلوبیت کمتری داشتند.
نتیجه‌گیری: نتایج نشان می‌دهد که بین شناسایی ژن‌های gyrA و parC و مقاومت سطح بالای به سیپروفلوکساسین ارتباط وجود دارد. بااین‌حال، برای شناسایی جهش‌های اختصاصی و تعیین ارتباط آن‌ها با فنوتیپ مقاومت، لازم است نواحی تعیین‌کننده مقاومت به کینولون (QRDRs) توالی‌یابی شوند.

Objective
One of the important causes of urinary tract infections (UTIs) is Enterococcus faecalis. This is especially true in hospitalized patients. It has been shown that ciprofloxacin resistance in E. faecalis can be caused by changes in the chromosomal genes encoding DNA gyrase and topoisomerase IV, as well as other resistance mechanisms. Therefore, the aim of this study was to determine the ciprofloxacin susceptibility profile of E. faecalis isolates isolated from urine samples. In addition, the identification of gyrA and parC genes associated with quinolone targets was the next aim of the study.
Materials and methods
Initially, 400 urine samples were collected from three hospitals in Baghdad. Among these 400 samples, 95 isolates were identified as Enterococcus faecalis. Among these 95 isolates, 20 (21.1%) were resistant to ciprofloxacin. VITEK 2 system and PCR were used for confirmation. MICs of ciprofloxacin were evaluated based on CLSI recommendations. gyrA and parC were subjected to amplification, and in silico docking was performed to examine ciprofloxacin interaction with DNA gyrase and topoisomerase IV.
Results
Ninety-five isolates of Enterococcus faecalis were identified from 400 urine samples. Twenty isolates (21.1%) showed ciprofloxacin MICs ranging from 64 to 512 μg/mL. While the rest (75 isolates, i.e. 78.9%) showed MICs between 2 and 4 μg/mL. The presence of gyrA and parC genes in the tested ciprofloxacin-resistant isolates was confirmed using PCR. The results show that the predicted docking score changed from -7.8 kcal/mol for the wild-type model to -3.89 kcal/mol for the mutant model. These changes indicate that a more unfavorable predicted interaction has occurred between ciprofloxacin and the mutant receptor. The results of docking analysis revealed that the predicted interactions of ciprofloxacin with the modeled mutant proteins, in comparison to the wild-type models were less favorable.
Conclusion
The results show that there is a correlation between detection of gyrA and parC genes and high-level ciprofloxacin resistance. However, to identify specific mutations and determine their association with the phenotype, it is necessary to sequence the quinolone resistance-determining regions.
